We’re heating things up with a give away (sweepstakes)!  On February 14, 2012, we’ll be giving away a Kindle Fire, the must have device for media consumption. 

Download books? Check.  Download movies? Check.  Download Magazines? Check.  Pretend to be reading the most current issue of Forbes, while really playing Angry Birds? Check. Check!  And who knew, it’s an educational device too!  Expand your vocabulary with a challenging game of Words with Friends. And to think, that all of this fits practically in the palm of your hand.  And here’s how you can win it:

It’s easy!  If you are fan on our Facebook page, you are automatically entered.  Not a Facebook fan, but would like to be one?  Well simply click on the box on the left to join us. Or, you can click here:  Like US! However, “liking” our page on Facebook, or even a Facebook account is not required to enter.  If you would like your name included in the drawing, but don’t have a Facebook account, please e-mail your request to join to winstoneye@hotmail.com.*  Leave your name (as it’s listed in our records), address, e-mail address and phone number and we will add you to the drawing.

On February 14, 2012, we will compile the names from all sources and draw the winner at random.  The winner will be announced shortly after, on our website.

Winner must be an established patient with any of our three locations. **

We must have a way to reach you. A current address (physical or e-mail) and at least 1 current telephone number on our records is required.  If your entry is based on your Facebook fan status, be sure your name matches the name we have on record at Winston Eye Associates.***   We will make every possible attempt to notify the winner by phone and by mail.  However, if the winner cannot be contacted, or if we receive no response within a reasonable time period, a forfeit will be declared, and a new winner will then be drawn. 

The winner must come to a designated Winston Eye Associates location to receive the prize.

The winner, by acceptance of the prize, authorizes Winston Eye Associates to use their name and/or image in this and future promotions (print, online or in-office).   Any winner that is drawn, that does not agree to this has the right to decline the prize.  A new winner will then be drawn.

Once the winner is declared, and the prize has been awarded, the winner releases Winston Eye Associates from responsibility from damages (personal injury or otherwise) of any kind resulting in the use of this device.  The winner agrees to direct any and all concerns regarding this product to Amazon.
